Guidelines for Submitting Public Records Requests

Please click the link below to obtain our Public Records Request form. Note that this will appear in a DocAccess viewing window, in order to assist any of our community members that may have Accessiblity needs. Please do NOT attempt to fill out the form in the DocAccess window. Please utilize the blue "Download Original PDF" button to download the form and fill out offline.

Your Public Records request may also include a request for your Protected Health Information (see Public Records Procedures link above). The PHI Authorization Request form is available  in the link below. As with the Public Records Request form, this will appear in a DocAccess viewing window, in order to assist any of our community members that may have Accessiblity needs. SUBMISSIONS

There are several ways to submit your Public Records request. Completed forms may be scanned and emailed to the Public Records Officer, they may be faxed to (253) 925-8889, you may utilize the Submit button below, or you may deliver to:

Pierce County Fire District 13, Attn: Public Records, 4815 Wa Tau Ga Ave NE, Tacoma, WA  98422-1757.
